Quadax makes every effort to keep the Quadax Portal secure, including 128-bit encrypted Secure Socket Layers (SSL). However, we recognize that malevolent forces exist, so Quadax cannot be responsible for damages resulting from loss of security.
A working e-mail address is required to register for, and to use, the Portal and our online applications. Quadax performs an e-mail verification test upon registration, but it is the client's responsibility to maintain reliable mail connectivity.
Quadax strongly recommends that users treat their e-mail login passwords as confidential. If you experience a problem with your login ID or password, or if you believe your password has become insecure, you must report it to Quadax immediately and obtain a new password.
Quadax reserves the right to disable a login ID, particularly in the event we believe the account has become insecure, the business contract has ended, or Site use has ceased. (HIPAA §164.312(a)(2)(i) & 164.312(d))
Quadax's Web site does not collect visitors' names, e-mail addresses, or other personally identifiable information, except as requested by a visitor when filling out a form. Quadax sends e-mails to clients based on the type of messages selected in their user profile in the Quadax Portal. Patient information ("PHI") is restricted to use by their care provider and is encrypted with 128-bit keys.
Web browsers request proxy IP addresses and browser and operating system versions and send the information to our Web servers. We use it to customize our Site's content to our visitors' technology. Cookies are not used to identify individual people or computers, except by the visitor's option and to preserve sessions when clients cross over web applications. Banners, where used, advertise only Quadax products and are tracked only by Quadax. Visitors' behaviors are not sold to third parties but are tracked internally to identify site functionality issues. We reserve the right to use Web Bugs for content we generate, for the purposes of privacy (as required by HIPAA), copyright enforcement and support.
Quadax adheres to the Safe Harbor Agreement between the U.S. Department of Commerce and the European Commission and Switzerland with respect to personally identifiable information that is transferred from the European Economic Area and Switzerland to the United States.
